This full time position is an exciting opportunity for a motivated self started with 2 years experience working with the frail elderly. Candidates will have experience with assessing and coordinating the homecare needs of the frail elderly populations and identify and develop specific plans of care in combination with the IDT. Candidate will also perform duties in accordance with existing Federal and State regulations. Scheduling will also be a requirement of the position.
